Terms and conditions for Little Squids Passes (pack of 4)
In this Agreement, the terms “you” and “your” refer to the Account Manager (AM) who shall be responsible for payment of all tuition fees and costs associated with attendance at OMS. A “Music Class” is a time period in which a musical instructor provides instruction to a PP based on a general musical program, which is designed for students of PP's general age. A Music Class may include but is not limited to sing-alongs, dancing, discussion, and demonstration of musical concepts, and practice suggestions.
1. Passes. Purchase of Little Squid passes entitles PP to four (4) 45-minute Little Squid group classes.
2. Cancellation of Class by Account Manager. In the event that the PP is unable to attend the class with at least 24 hours notice of an absence, PP may use their pass for another class at some point in the future. Any cancellations under 24 hours will result in a forfeiture of that pass.
3. Cancellation of Class by OMS. In the case that OMS cancels the class, PP will receive a full credit and may use that credit for another class at some point in the future.
4. Refunds. There are no refunds provided for paid tuition.
5. Billing Authorization. I give OMS authorization to charge my card for this purchase.
6. Make-Up Policy. There are no make-ups for missed classes.
